Childbirth Classes

  • Infant, toddler, child nutrition counseling

Adult Health

  • Blood Pressure screening and monitoring
  • Includes communication with personal physician
  • Medication management assistance
  • Home visits for chronically ill or disabled who do not qualify for Home Health Services for nursing assessment and medication monitoring only
  • Functional assessments for nursing home placement and home care
  • Community Based Services Waiver
  • Diabetic Support Groups at Douglas and Glenrock Senior Citizen Centers

General Services
